The Care Coordinator is responsible for managing and coordinating all aspects of client care to ensure high-quality service delivery and client satisfaction.
This includes overseeing onboarding, staffing, case compliance, and daily operational issues reporting directly to the Director of Care Coodination.
The role requires strong organizational skills, proactive communication, and a commitment to ensuring that every client receives consistent, authorized care.
The Coordinator serves as the ongoing primary point of contact for their assigned clients from case acceptance through long-term service retention.Key ResponsibilitiesClient OnboardingEnsure all onboarding documentation is completed and admission agreements are signed before activating servicesCare Coordination and StaffingManage, maintain, and optimize staffing schedules to fill 100% of authorized service hoursRespond promptly to aide call-outs, resignations, or client dissatisfaction to prevent service disruptionsRecruit new caregivers as needed to ensure coverage and prevent open shiftsCollaborate with vendors and third-party service providers to meet client care needsClient Case ManagementKeep client records current with updated demographics, preferences, care plans, and incident trackingProcess authorization updates and adjust staffing accordinglyManage discharges with proper documentation and within compliance timelinesEnsure scheduling of caregiver shifts is accurate at all times, with correct times and assignments reflected in the systemMonitor and clear out prebilling daily to ensure all visits are verified and ready for timely invoicing and payroll processingClient Satisfaction and CommunicationServe as the primary liaison between assigned clients and CareSphereCommunicate clearly, respectfully, and frequently to address client questions or concernsFollow up on leads and status changes, supporting long-term retentionCompliance and DocumentationEnsure that all client documentation is complete, accurate, and compliant with internal and external standardsEnter all client communications, updates, and incidents into the appropriate systems in real timeOperational OversightManage and resolve daily items including Promptly Pending Contracts, Requested Cases, Open Time Off Requests, Open Timesheets, and Open Fill-in ShiftsMaintain strong relationships within your territory to drive lead generation and referralsRequirementsHuman Relations:Uphold and model a professional office culture at all times, ensuring team adherence to dress code standards, respectful communication, and a positive, solutions-oriented attitude in all interactionsBuild and maintain professional and empathetic relationships with clients, caregivers, and external partnersCollaborate effectively with other team members and departments to ensure smooth operationsHandle sensitive situations with patience, discretion, and professionalismQualifications:Adaptable to a fast-changing, entrepreneurial environmentStrong organizational and time-management skillsExcellent communication skills—both verbal and writtenConflict resolution and problem-solving abilitiesProficiency in Microsoft Office and scheduling platformsEducation and Experience:Previous experience in client services, healthcare coordination, or case management preferredFamiliarity with healthcare authorizations and staffing models is a plusAdditional Notes:This is a fast-paced, mission-driven position that requires a balance of people skills and operational excellenceThe Care Coordinator plays a critical role in ensuring client retention and satisfaction, as well as driving operational success in their assigned regionOpportunities for growth exist within the organization for high performersSchedule:Monday: 4 PM to 10 PM ESTTuesday: 4 PM to 10 PM ESTWednesday: 4 PM to 10 PM ESTThursday: 4 PM to 10 PM ESTSunday: 8 AM to 10 PM ESTBenefitsCompetitive pay in US DollarsYou'll get to work with some amazing, respected business professionalsWe really value and encourage input from our team membersWe offer different payment methods (Zelle, Face Bank, Binance)You'll get paid every two weeksBirthday and work anniversary celebrationsAlliances with many brands grant extra benefits to freelancersWeekly recreation dynamics that foster a healthy work environment
