Unified Care Hospice is seeking a dedicated Registered Nurse Case Manager to provide compassionate, patient-centered care over the full weekend shift from Friday at 5 PM to Monday at 7 AM.

We are a team united by compassion, dedicated to making a meaningful difference in the lives of those we serve.Responsibilities:Perform comprehensive patient assessments and develop individualized care plans.Manage hospice care, ensuring alignment with patient goals and the hospice philosophy.Provide skilled nursing care, pain and symptom management, and emotional support.Coordinate with physicians, social workers, chaplains, and the interdisciplinary team.Educate patients and families about the hospice process and symptom management.Document care and monitor patient progress using electronic medical records.Participate in team meetings and contribute to the improvement of patient care delivery.Qualifications:Hospice or Home Health experience preferred.Strong communication and interpersonal skills.Reliable transportation for patient visits (mileage reimbursement available).Schedule:Full Weekend: Friday at 5 PM – Monday at 7 AMCovers all PRN visits, admissions, and scheduled visits during the shift.Join Unified Care Hospice, where your work is more than a job—it's a mission to provide comfort, dignity, and compassionate care to those in need.
