Bring your passion and commitment and make a difference to those that matter – the next generation.As the Care Navigator, you will provide navigation, clinical consultation and coordination for CDS consumers, and interface with developmental and disability supports and services, including the National Disability Insurance Scheme (NDIS). Your role will include sharing knowledge to CDS staff regarding disability and other agency access, eligibility, supports and services. You will liaise with other agencies, community organisations and service providers to strengthen collaboration, information sharing and engagement with CDS and support the transition of care between CDS and other service providers.About Child Development ServiceThe Child Development Service provides multidisciplinary assessment and intervention services to infants, children and young people with developmental delay or difficulties that impact on function, participation and/or the parent-child relationship. Clinicians adopt a child and family-centred approach, working in partnership with families to address developmental concerns that are of priority to the family.Our teams include Clinical Psychologists, Developmental Paediatricians, Nurses, Occupational Therapists, Physiotherapists, Social Workers, Speech Pathologists, and other allied health professionals. The Child Development Service provides services at approximately 18 locations across the metropolitan Perth area, as well as providing home, school and community visits as needed.Eligibility to ApplyAustralian citizenship or permanent residency is an essential requirement for applicants to be considered for permanent positions in the public sector.
