DescriptionOVERVIEW:The Salvation Army Housing Programs were developed to provide both Emergency Shelter and Transitional Housing for homeless families transitioning from homelessness to independence. This position is responsible for providing case management services, including Program Participant assessment, intake, and facilitation of program meetings. The employee is required to live on site to handle case management related emergencies for the Housing Department as needed.DUTIES AND RESPONSIBILITIES:Conduct phone screens and interviews with potential program participants for all housing programs.Develop and implement case plans, providing consistent individual case management in accordance with TSA and program standards.Maintain and update all program files, referrals, and case management notes.Communicate with and encourage program participants to maintain compliance with program requirements and advance towards their case plan goals.Ensure program compliance and integrity for program participants.Maintain participant confidentiality in accordance with the law and TSA policy.Input required data into the Social Services and HMIS databases.Maintain a working wait list for potential program participants.Enforce program policies and disciplinary processes, including write-ups for policy violations and terminations with Director approval.Perform unit inspections to ensure program compliance in safety and sanitation.Facilitate emergency shelter house meetings and assist with program workshops and transitional housing meetings as needed.Participate in and assist with Housing Committee meetings.Organize and assist with program events for participants, including Family Nights and holiday parties.Collaborate with community agencies and attend community social service agency meetings as needed.Assist with the collection, recording, and reporting of program statistics on a monthly basis.Report to Housing Manager, Director, and Corps Officers with regular program updates.Perform urine analysis tests on program participants as needed.Contact program participants who have transitioned out of the program for follow-up services and assist with Alumni programs and events.Assist Housing Manager with scheduling and supervising volunteers and interns.Perform program duties in the absence of the Program Assistant or Housing Manager as appropriate.Be available on call for emergency situations related to case management issues.Be on-call for TSA Housing program emergencies one weekend each month.Provide transportation to program participants in TSA vehicles as needed.Other related duties as required.EDUCATION AND EXPERIENCE:High school diploma or equivalent (required).CA Driver's License (required).Bachelor's Degree in Social Sciences or equivalent education and experience.Experience with Social Services agencies and service providers within the community.SKILLS AND ABILITIES:Able to confidently meet and relate to the public.Able to work positively with all staff members.Strong written and verbal communication skills.Ability to demonstrate consistency.Ability to work with a diverse community.Understanding, compassionate, yet firm.Great organizational skills.Willing to take initiative and be a self-starter.Capable of maintaining a high level of integrity and confidentiality.Knowledge of confidentiality laws pertaining to program participants.Knowledge of computer software programs.PHYSICAL REQUIREMENTS:Ability to lift up to 25 lbs.
